Health Risk Assessment Available Today for Faculty and Staff

All benefits-eligible employees may take the Health Risk Assessment staring today. Employees have until November 17 to complete the online survey as part of the Annual NUFlex enrollment in Firefly Employee Self Service. The HRA survey must be completed each year in order to receive the enhanced wellness and preventive services benefits for the following calendar year.
Faculty and Staff who are getting blood work done at the Health Fair on Nov. 2nd may wish to keep that information handy when filling out the Health Risk Assessment during the annual NUFlex enrollment, which runs from Oct. 30 to Nov. 17.
The Health Risk Assessment (HRA) program is part of the Annual NUFlex enrollment in Firefly Employee Self Service. Participation is voluntary; however, by completing this survey, you will receive a personal health report that will help you assess and monitor your personal health status. Employees who are enrolled in the University’s Blue Cross Blue Shield medical plan and complete the HRA will also be eligible for enhanced wellness and preventive services benefits for themselves as well as their covered family members. Enhanced wellness and preventive services include:
- Increased annual adult preventive care allowance from $250 to $300
- Increased dependent child (under age 2) preventive care allowance from $500 to $600
- 100% coverage for routine preventive colonoscopy once every 10 years beginning at age 50 (Services must be provided by a Blue Cross Blue Shield PPO Provider)
- $0 co-pay for generic prescription drugs through CVS Caremark’s mail service only
You will receive a confirmation email following your successful completion of the HRA survey. The confirmation will be sent to the email address you provide when completing the survey. We still recommend you print or save the HRA report, but the email will be your proof that you completed the assessment.
