Reporting Incidents or Injuries of Personnel
- Contact your PI/Lab Manager
- For medical care, report to Nebraska Medicine-UNO Health Center, H&K 102, or your primary physician
- For after hours care: Visit one of Nebraska Medicine’s Immediate Care Clinics or Emergency Services.
- Staff or Student Employees (Federal Work Study, Student Worker): Submit an incident report to EHS
- Non-employee students or volunteers: Submit a report to the Behavioral Review Team
Biological exposures:
- Follow the instructions given to you by your PI/Lab Manager
- Notify EHS
- Follow IBC Post-Exposure Algorithms and report the incident to IBC within 24 hours.
Reporting Concerns Regarding the Care, Treatment, and Use of Animals
Concern for the welfare of animals used in research and teaching is the responsibility of all members of the University community. Individuals may report concerns at any time through various methods and, if preferred, may do so anonymously.
The purpose of reporting animal welfare concerns is to ensure that they are fully addressed by the institution as they occur. Timely reporting allows the institution to provide the highest quality animal care by engaging all available resources.
UNO shall attempt to maintain the confidentiality of an individual who reports concerns or misconduct upon request. However, confidentiality cannot be guaranteed, and disclosure of the individual's identity may be necessary in order to fully investigate the concern. There is no retribution or discipline for anyone who reports a concern in good faith.
