Constitution and Citizenship Day
UNO annually prepares events for Constitution Day, which is normally observed on September 17, commemorating the September 17, 1787 signing of the Constitution. UNO is required to hold an educational program on the United States Constitution annually around Constitution Day.

Cost of Attendance
The cost of attendance is the estimated full and reasonable cost of completing an academic year as a student attending UNO. Federal Work Study
Since 1969, UNO has participated in the Federal Work Study program. This program provides employment opportunities to students as part of a student's financial assistance package. Eligibility for Federal Work Study is based on demonstrated financial need and position availability. Officially Withdrawing from UNO
Students wishing to drop or withdraw from a course or entirely withdraw from all enrollment at UNO should withdraw their enrollment on MavLINK. Requests to drop a class submitted via fax or U.S. mail will be processed based on the dates appearing on the fax or U.S. mail postmark. Treatment of Title IV Aid When a Student Withdraws
When a financial aid recipient entirely withdraws from UNO (withdrawn from all courses), a portion of aid received may need to be returned to the appropriate financial aid programs. Federal financial aid funds are awarded to a student under the assumption that the student will attend school for the entire period for which the assistance is awarded. The amount to be returned varies upon many factors, including but not limited to types of aid received, withdrawal date, and amount of institutional charges. Studying Abroad
A student's enrollment in a program of study abroad approved for credit by UNO may be considered enrollment at UNO for the purpose of applying for financial aid assistance under the Title IV, HEA programs.
All students applying for financial aid and planning to apply for eligible grants, scholarships, or loans for the cost of their study abroad program should complete an Education Abroad Financial Support and Scholarships Form, which can be obtained from the Education Abroad Office. The student and Education Abroad Office will review the form and determine the amount of UNO or transferable credit hours that will be earned as part of the study abroad. You will need to return the completed form and any required documentation to the Office of Financial Support and Scholarships. The Office of Financial Support and Scholarships uses the Education Abroad form to determine if the student's cost of attendance can be increased and to ensure earned credit hours abroad can be counted as countable enrollment for financial aid purposes.

Transfer Credit Policies
The transfer credit policies include the criteria UNO uses regarding the transfer of credit earned at another school, and a list of schools with which UNO has established an articulation agreement.
The Transfer Articulation Guide can be used to see how coursework typically transfers to UNO. UNO has established institution wide-general education agreements, general education guides, and Associate-to-Bachelor (A-to-B) Program-Specific Transfer Agreements.
In order to award transfer credit, an official, sealed transcript (or official electronic transcript) from each college/university previously attended needs to be sent to the Office of Undergraduate Admissions.

UNO has pre-enrollment health requirements that must be satisfied prior to enrolling in courses. All admitted students are required to submit an official copy of required immunization records prior to enrolling in courses.

Textbook Information
Textbook information, including the International Standard Book Number (ISBN), if applicable, is available in the MavLINK student portal or on the UNO Bookstore website.
To view textbooks in MavLINK, log in and under "My Classes", select "View All Books".
You are eligible to charge your books and supplies from the UNO Bookstore to your student account, with a maximum amount of $2,000 per semester. There is no financial aid requirement in order to charge books to your student account; however, you are responsible for all charges accrued on your student account. In order to charge UNO Bookstore purchases to your student account, ask for this payment option at the cashier station of the UNO Bookstore. Your charges will be billed at the end of each month and are due by the 23rd of the following month.
Please note, financial aid disbursements will not pay for UNO Bookstore books or supplies unless authorized by you when reviewing your financial aid on MavLINK.

Teacher Preparation Report
UNO provides a Teacher Preparation Report annually to the state and general public. UNO participates in programs that prepare teachers for initial and renewal State Certification. This report is also known as the Title II report and is provided to the Nebraska Department of Education which provides copies of the report via its website to the general public.

UNO, in accordance with Federal regulations, has established a Satisfactory Academic Progress (SAP) policy, which establishes standards that students must maintain in order to be considered to be making satisfactory progress in his or her program for the purpose of receiving financial assistance. Alternative (Private) Student Loans
Alternative (private) student loans are loans used for educational related expenses for students attending an institution of higher education. They are often used to bridge gaps in financing between Federal Title IV financial aid offered and the cost of attendance.
If you are a dependent student or graduate student and need additional funding to meet college costs after your eligibility for federal grants and federal loans has been considered, it is important to consider the differences between Direct PLUS loans and alternative (private) student loans.
Terms and conditions of Title IV, HEA Program loans may be more favorable than the provisions of alternative (private) education loans.
The UNO Office of Financial Support and Scholarships does not require the completion of a Free Application for Federal Student Aid (FAFSA) but strongly encourages students to complete a FAFSA in order to apply for and utilize eligibility in Federal and State financial assistance prior to applying for an alternative (private) loan.
Filing a FAFSA will determine your eligibility for grants (assistance that does not have to be repaid) and other need-based aid, such as Direct Subsidized Stafford loans (available to undergraduate students, interest is paid by the government while you are enrolled in school at least half-time and during deferment periods). Regardless of the demonstrated financial need on your FAFSA, you may be eligible for a Direct Unsubsidized Stafford loan (interest accrues while you are attending school).

Preferred Lender Arrangements
The UNO Office of Financial Support and Scholarships does not participate in preferred lender arrangements.
The UNO Office of Financial Support and Scholarships will work with any lender that participates in alternative (private) education loans. You may choose any alternative (private) education loan lender that you wish.
Students should contact prospective lenders with any questions during the application process. You may be required to submit documentation as part of the application process. As applicants of alternative (private) loans are subject to a credit check, the lender will let you know the outcome of the credit check, if a co-signer is necessary in order to approve the application, as well as inform you of any required documentation that needs to be submitted.

Self-Certification Form (Alternative Loans)
If you apply for an alternative (private) student loan, as part of the application process you must submit the Private Education Loan Applicant Self-Certification form to your lender. The Higher Education Opportunity Act of 2008 (HEOA) added section 128(e)(3) to the Truth In Lending Act to require that before a private educational lender may consummate a private education loan for a student in attendance at an institution of higher education, the private education lender must obtain the completed and signed Self-Certification Form from the applicant.
Upon request, the Office of Financial Support and Scholarships will assist you in completing Section 2 of the Self-Certification Form. This section requests information about your cost of attendance (COA), any estimated financial assistance (EFA) for the period of enrollment covered by the loan, and the difference between the COA and EFA.

Public Service Loan Forgiveness (PSLF)
The Public Service Loan Forgiveness (PSLF) Program forgives the remaining balance on your Direct Loans after you have made 120 qualifying monthly payments under a qualifying repayment plan while working full-time for a qualifying employer.
UNO is a qualifying employer for Public Service Loan Forgiveness (PSLF). If you are a full-time employee of UNO and want payments on your Direct Loans to be considered for PSLF, please complete the Public Service Loan Forgiveness Employment Certification Form and submit it to the UNO Office of Human Resources for processing.
If you think you might be eligible for Public Service Loan Forgiveness (PSLF) now or in the future, complete and submit the Employment Certification form as soon as possible. In order to ensure you are on track to receive PSLF, you should continue to submit the form both annually and every time you switch employers.

Entrance Counseling
First-time borrowers of Federal Direct Stafford loans and Federal Graduate PLUS loans must complete Loan Entrance Counseling. UNO utilizes the U.S. Department of Education's Entrance Counseling online on StudentAid.gov to fulfill entrance counseling requirements.
Entrance counseling is a one-time requirement that must be completed prior to a Federal Direct Stafford loan or Federal Graduate PLUS loan can be disbursed. Entrance counseling takes about 30 minutes to complete and must be completed in one session.
During entrance counseling, you will learn about what a Direct Loan is and how the loan process works, managing your education expenses, other financial resources to consider to help pay for your education, and your rights and responsibilities as a borrower.
Parents borrowing a Federal Direct Parent PLUS Loan to help pay for their children's education expenses do not have to complete entrance counseling.

Exit Counseling
Borrowers of Federal Subsidized, Unsubsidized, or PLUS Loans under the Direct Loan Program or FFEL Program must complete exit counseling each time the borrower drops below half-time enrollment, graduates, or leaves school. Exit counseling provides important information to prepare borrowers to repay their federal student loan(s). UNO utilizes StudentAid.gov to fulfill exit counseling requirements.
Exit counseling takes approximately 30 minutes to complete and must be completed in one session. Borrowers will need their verified FSA ID, details on income, financial aid, and living expenses, contact information for next of kin, two references who live in the United States, and future employer information (if known).

PLUS Credit Counseling
Borrowers of Federal Direct Parent PLUS Loans and Federal Direct Graduate PLUS Loans are subject to a credit check of adverse credit history by the U.S. Department of Education.
If a borrower is found to have an adverse credit history, the borrower may still borrow a PLUS loan if an endorser is obtained who does not have an adverse credit history and meets other eligibility requirements. Borrowers may also be able to appeal the adverse credit decision with the U.S. Department of Education.
If a borrower successfully obtains an endorser or appeals the adverse credit history decision and wants to borrow a PLUS loan, PLUS Credit Counseling must be completed in order for the Federal Direct PLUS loan to be eligible for disbursement. PLUS Credit Counseling is an online information session for PLUS borrowers. Once the PLUS Credit Counseling session is completed, applicants will be provided a financial summary that includes loan balances, income expenses, and repayment plan details based on information provided during the counseling session. Note that PLUS Credit Counseling is only required for applicants found to have an adverse credit history who have obtained an endorser or appealed their adverse credit decision; it is optional for borrowers initially found not to have an adverse credit history. PLUS Credit Counseling must be completed on StudentAid.gov.

This includes providing information on preventing drug and alcohol abuse. CAPS provides substance use evaluations, substance use disorder counseling, and treatment for co-occurring disorders. UNO also provides tobacco cessation counseling, court/probation referred substance use counseling and provides an open Alcoholics Anonymous meeting on campus.

Campus Drug Policies
UNO has established a Campus Drug Policy that provides standards of conduct for employees and students regarding alcohol and drugs, a description of applicable legal sanctions under Federal, State, or Local law for unlawful possession or distribution of illicit drugs and alcohol, a description of health risks associated with the use of illicit drugs and alcohol, and assistance options, university sanctions, and additional information. The campus drug policy is annually distributed via email. New students/employees receive information at Orientation sessions for new students or at New Hire Orientation for new employees. The Campus Drug Policy is reviewed at least biennially.
Human Resources has a Drug-Free Workplace policy as required by the Drug-Free Workplace Act of 1988.

UNO Faculty and Staff Employee Assistance Program (FSEAP)
Available to benefit-eligible UNO Faculty and Staff and their eligible dependents, the UNO Faculty Employee Assistance Program provides confidential counseling and referral services.
Common reasons an individual may choose to seek assistance through the FSEAP include marital/relationship issues, alcohol and drug abuse/dependence, work/life balance, personal wellness, family/parenting concerns, grief and loss, stress, mental illness, depression/anxiety, divorce, and financial concerns. Faculty and staff do not need a referral from a manager or supervisor to use these services. UNO works with Best Care EAP to administer the FSEAP.

Family Educational Rights and Privacy Act (FERPA)
UNO Procedures Concerning Student Records/Safeguarding Consumer Information
The Family Educational Rights and Privacy Act (FERPA) afford eligible students certain rights with respect to their educational records.
UNO offers an online training session designed to prepare authorized users of the campus community to fully understand the responsibilities of handling student record information under FERPA, and the university's student records policy.
UNO has created procedures concerning student records in accordance to FERPA - also known as the Buckley Amendment. This includes what is defined as public or confidential information, limitation of access, custodians of student records, waivers of right of access to confidential recommendations, and consent of disclosures of personally identifiable information.
In order to release information and discuss items such as academics, financial aid, personal information, student accounts, and student records to parents and family members, there is an electronic process that must be completed to authorize access.

HIPAA and FERPA
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) applies to health care providers, private benefit plans, and health care clearinghouses. It does not apply to other types of organizations whose receipt or maintenance of health records is incidental to the normal course of business. FERPA does not limit what records a school may obtain, create, or maintain. It provides safeguards for education records.
The Accessibility Services Center (ASC) normally obtains and maintains health records for each student who applies for services or waivers. If a health record is used to make a decision in regard to a student's education program, the health record may be construed to be an education record. In that case, the normal FERPA provisions for safeguarding the record would apply.
The Accessibility Services Center (ASC)'s handbook provides information about the confidentiality and the release of information and retains student records in compliance with HIPAA and FERPA.

Clery Act (Campus Security)
Annual Fire and Safety Report
On an annual basis, the UNO Public Safety Department prepares a security report that reflects current policies, including information about the student code of conduct, crime statistics, timely warning reports, drug and alcohol policies, and the missing student policy. Emergency response information and policies and procedures regarding alleged dating violence, domestic violence, sexual assault, and stalking are provided. The annual security report is submitted to the U.S. Department of Education. For further information about Clery Act required disclosures and Public Safety policies please read the annual report.
Title IX is a comprehensive federal law that prohibits discrimination on the basis of sex in any federally funded education program or activity.

Daily Crime Log
The Clery Act requires institutions that maintain a campus police or security department to maintain a written, easily understood daily crime log that records, by the date the crime was reported, any crime that occurred on buildings and property that are part of the institution's campus, the institution's noncampus buildings, and property and public property within or immediately adjacent to and accessible from the campus. This also includes areas within the patrol jurisdiction of the Public Safety department.

Timely Warning and Emergency Notifications
During an emergency, UNO utilizes a number of resources to disseminate critical information and instructions during an emergency. Students, faculty, and staff are able to enroll in text message and email alert capabilities.
The federal Clery Act requires UNO to issue timely warnings to the campus community when there is an ongoing threat to public safety.
UNO typically will not issue a timely warning if a crime is made and it is believed that a threat no longer exists.

Equity in Athletics Report
The Equity in Athletics Disclosure Act requires institutions that participate in Title IV, Federal student assistance programs and have an intercollegiate athletic program to prepare an annual report to the Department of Education on athletic participation, staffing, and revenues and expenses, by men's and women's teams.
The Equity in Athletics Report is prepared by the UNO Athletics Compliance Office and is made available to the public by October 15 of each year. UNO submits the report annually to the U.S. Department of Education. Students, prospective students, and the public can view the Equity in Athletics Report upon request.

Student Right-to-Know Act
Enrollment Information, Retention and Graduation Rates, Financial Aid Related Data, and More
UNO submits information to the National Center for Education Statistics (NCES), which is the primary federal entity for collecting and analyzing data related to education. From this data, the U.S. Department of Education publishes items on the College Navigator website for UNO. The College Navigator website offers quick and easy access to data on tuition/fees, enrollment, faculty, graduation rates, campus safety, and other important topics.
UNO also participates in the Common Data Set (CDS). UNO follows a standard set of data elements and cohort guidelines when completing the annual CDS survey. CDS is optional, but UNO participates and publishes the CDS survey annually. The CDS includes information on Enrollment and Persistence, First-Time, First Year (Freshman) Admission, Transfer Admission, Academic Offerings and Policies, Student Life, Annual Expenses, Financial Aid, Instructional Faculty, Class Size, and Degree Conferred.
