International Students

To ensure the public health of our campus community, all enrolled international students are required to:

• Submit a copy of your immunization records showing two measles, mumps, and rubella (MMR) vaccinations or a positive rubeola titer
• Receive a tuberculosis test at Nebraska Medicine - UNO Health Center when you arrive

This requirement must be completed soon after arrival to avoid having a hold placed on the student account that prevents you from signing up for future classes.

Tuberculosis Test

The tuberculosis test must be given at the UNO Health Center—no exceptions. After your test, you will receive a letter or phone call informing you of your result. If the test is positive or indeterminate, you must have a chest X-ray and follow up with a provider at the UNO Health Center.

Immunization Records

MMR immunization records must:

• Show proof of two separate MMR shots showing month, day, and the year it was given OR students can submit a copy of a positive rubeola blood titer lab result showing immunity. The first shot must have been received no earlier than a student’s first birthday, and the second shot must have been received at least 28 days later.
• Include the full legal name of the student and the student’s birthdate.
• Be in English

MMR immunization records can often be found at:

• Previous high school in your home country
• Previous university/college
• Family doctor or local hospital in your home country

If you haven’t been vaccinated or cannot find your immunization records, you can receive the MMR immunizations or have a rubeola blood titer test drawn at the UNO Health Center.

Please note tuberculosis tests, MMR immunizations, and laboratory blood titer services will incur a charge and are the responsibility of the student. These charges can be billed to the student insurance plan or other designated domestic insurance policies.

Submit Your Records

International students can submit their immunization or rubeola lab test result records by bringing them to their tuberculosis test appointment at the UNO Health Center in 102 H&K, or uploading them to MavLINK.

Remember, the form cannot be submitted until the relevant immunization record is attached to the form. However, the form can be saved until a later date when students are ready to attach the relevant immunization record to the form and submit it.

Immunization Waiver

Students who are unable to receive vaccinations due to religious or medical reasons may fill out this waiver form. The form must be filled out in its entirety, be notarized, and then be uploaded to MavLINK in place of immunization records as explained above.

Optional: Power of Attorney (POA) Form (for 18-year-old students)

The age of majority in Nebraska is 19, meaning parental permission is required before a student 18 or younger can receive health care at Nebraska Medicine UNO Health Center. It is strongly recommended that parents or guardians of 18-year-olds fill out and have notarized this Power of Attorney form.

POA forms can be dropped off, mailed, or faxed to the UNO Health Center front desk, located at 102 H&K. For more information, call the UNO Health Center at 402.554.2374 (Please dial the full 10-digit number whether on or off campus.)

Please note POA forms are not valid for 17-year-olds.

If you are unable to complete a POA form, the UNO Health Center staff can call your parent or guardian at the time of your appointment for consent to treat. Translation services are available for no cost.
